Abstract
CAPD(Continuous Ambulatory Peritoneal Dialysis) is an effective replacement treatment for the patient with end stage of renal disease. Peritonitis is the most common complication with CAPD patients and a leading cause of CAPD failure. Authers reviewed the experience of antibiotics for the treatment of peritonitis.
Clinical study by antibiotics using intraperitoneal therapy were carried out on 20 patients of peritonitis among the patients of peritonitis among the patients who have been associated with CAPD since 1988 in Korea University Hospital. The average age of these patients was 50.9 years and the percent ratio of male and female patients were 70% and 30%. The incidence of peritonitis was 1.7 episodes/patient-year.
The peritonitis were occurred 45% within a month and 85% a year after starting the CAPD. An initial antibiotics given by intraperitoneal route was 65.8% using the mixture of the 1st generation cephem and aminoglycoside. Complete recovery after 1st step care was 66.7%. The mixture of the 3rd generation cephem and aminoglycoside as replacement of the above prescription of which was used most commonly rating 36.4% and vancomycin next rating 27.3% after the failure of 1st treatment. In this case, the response rate of treatment was 42.8%.
Treatment failure was resulted to 40% of catheter removal, 20% antibiotics therapy and 2 death cases. In summary, 81.5% of after all peritonitis were cured by antibiotics therapy, but 14.3% catheter removal and 4.1% death, respectively.
